Fertility treatments and services include

Assisted reproduction

Assisted reproduction refers to processes in which eggs or sperm are treated outside of the body. Commonly used techniques include:

  • In vitro fertilization (IVF): IVF is the most commonly used type of assisted fertilization and has a good success rate. Fertility medications are given to the woman which stimulate the ovaries to produce multiple eggs, as well as preventing premature ovulation. When the eggs are ready for retrieval, another medication is given which triggers ovulation. Under anesthesia, the eggs are retrieved from each follicle (also called egg collection or ovum pick up) via a needle which is guided by an ultrasound probe. Once the eggs have been retrieved, they are combined with sperm so that fertilization can take place and an embryo be produced (embryo culture). The embryo is then placed back into the uterus (embryo transfer) where it implants. If more than one embryo is transferred, the pregnancy may result in multiple births.
  • Intrauterine insemination (IUI): A less invasive procedure than IVF, IUI involves inserting sperm directly into the uterus via a small catheter. This increases the number of healthy sperm reaching the fallopian tubes. The procedure is performed during the woman’s most fertile stage, increasing the chances of fertilization.

Egg freezing

As a woman ages, so do her eggs. From the age of 35, the quantity and quality of eggs decline more rapidly, reducing the chances of conceiving naturally. The likelihood of miscarriage is increased and so are the chances of the child being born with certain genetic abnormalities. By freezing eggs in advance, a woman can choose when she wants pregnancy to occur. It is also used by women who need to undergo treatment that may affect their fertility. The process of egg freezing involves taking medications which stimulate ovulation. Once the eggs are mature, they are retrieved and frozen. When the woman decides she wants to become pregnant, the eggs are thawed and fertilized via an intracytoplasmic sperm (ICSI) injection.

Intracytoplasmic sperm injection (ICSI)

ICSI involves injecting a single sperm directly into an egg in a laboratory. A healthcare professional, called an embryologist, selects a healthy and active sperm and injects it into the egg’s cytoplasm. ICSI is a procedure used when the male’s sperm count is low, or the sperm quality is not good. It is also used when sperm have been retrieved surgically or following unsuccessful IVF cycles. Following ICSI, the embryo is monitored for signs of fertilization. If successful, the embryo is transferred back into the uterus or frozen.

Preimplantation genetic testing (PGT)

PGT allows embryos produced following IVF to be genetically tested before they are transferred back into the uterus. After fertilization, a few cells are removed from the embryo and comprehensive chromosomal screening looks at all 24 chromosomes. The screening can identify the baby’s gender, as well as any missing, additional or abnormal chromosomes, a major cause of miscarriage. The embryo can also be tested for any known hereditary or genetic diseases. PGT also allows human leukocyte antigens (HLA) matching, a procedure that is used to select embryos that are compatible with a child needing cord blood or bone marrow transplant.

Sperm freezing

Sperm can be frozen for future use if a man wants to preserve his fertility or if he needs to undergo treatment which may affect the sperm. Once the sperm has been collected, it is analyzed to check its motility, morphology and quantity, and then frozen. When needed, the sperm is thawed and used in IUI, IVF or ICSI procedures.

Infertility can be an emotional journey that impacts the psychological wellbeing of men and women. Alongside medical fertility treatments, it is very important to receive emotional support from trained professionals at every stage. Counselling sessions encourage couples and individuals to be open about their thoughts and feelings. Counselling sessions also provide an opportunity for education on different procedures so couples can decide what is right for them. Genetic counseling is an important part of PGT procedures, where couples can discuss their medical history and the implication of future test results.
