Evaluating and diagnosing fertility concerns 

Diagnosing fertility issues usually begins with baseline assessments, which look at the hormonal and structural components of reproductive health. Evaluating and diagnosing fertility in women

Fertility in women involves the release of a healthy egg from the ovaries, the passage of the egg through the fallopian tubes, fertilization by sperm, travel to the uterus and attachment to the uterus wall. Diagnostic tests aim to identify problems with any of these steps, and may include

  • Hysterosalpingography: This involves injecting a special dye into the uterus which is visible on an X ray. It can identify blockages in the fallopian tubes and other problems with the uterus.
  • Hysteroscopy: A thin device with a light is inserted through the cervix and into the uterus to check for signs of disease or irregularities.
  • Imaging tests: An ultrasound of the pelvis can identify problems with the uterus or ovaries.
  • Laparoscopy: A small device with a camera is inserted through a small incision in the stomach, to look at the fallopian tubes, ovaries and uterus. It is used to diagnose conditions such as endometriosis, blockages, scarring and other issues. The procedure is also used to treat certain conditions, such as the removal of endometriosis tissue and fibroids.
  • Ovarian reserve testing: This involves hormone testing at the beginning of the menstrual cycle and can indicate how many eggs are present.
  • Ovulation testing: Hormone levels can be measured via a blood test to find out whether ovulation is taking place.
  • Thyroid function test: A blood test that can determine whether too little or too much thyroid hormone is being produced, which can affect fertility.

Evaluating and diagnosing fertility in men

Male fertility is dependent on the testicles making enough healthy sperm. Diagnostic tests aim to identify any problems with the quality and quantity of sperm produced, and include

  • Genetic testing: A blood test can identify genetic defects that may be causing infertility.
  • Hormone evaluation: A blood test measures the levels of hormones such as testosterone.
  • Imaging: An ultrasound image can reveal problems in the scrotum, the glands that make semen or the tubes which carry the sperm from the testicles.
  • Semen analysis: A sample of semen is tested to look at the quality and quantity of sperm.
  • Testicular biopsy: A small amount of tissue is taken from the testicle to see how well it produces sperm. A biopsy can also identify issues which may be preventing sperm from leaving the body.
